Online registration Instructions

As part of the initial onboarding process, complete this online registration form to exchange data with the American Samoa Department of Health Immunization Division (ASDoHID) Web Tama IZ Registry (WTIR).

Once you complete and register, your organization will be notified by ASDoHID that your organization is now on the integration list.
